New Hampshire Survey of Victims
Thank you for taking the time to complete this survey. Your feedback is
important and will help other victims in the future.
We use the term “victim” to mean: anyone who has suffered physical, emotional
or financial harm as the result of a crime or attempted crime, whether or not
it was reported to the police.
The survey is voluntary, and your answers are confidential and anonymous. You
may also skip any question you do not want to answer. The survey should take about 10-15 minutes to complete.
If you have any questions about this survey, or would like a paper version,
please contact Lindsay Gannon, at firstname.lastname@example.org
or by telephone at 207-767-6440 ext. 149.
